Report Overview
FreeStyle Auto-Assist Neo software presents six easy-to-read reports


1. Snap Shot

The Snapshot Report provides a data summary for a specified date range, including up to six notes relating to important blood glucose trends.
2. Modal Day

The Modal Day Report shows the daily pattern of blood glucose levels over a specified date range along with average total insulin doses, type of insulin used (basal or rapid-acting).
3. Daily Log

The Daily Log provides a table of blood glucose readings, results and other values, such as ketone test results and logged insulin doses for each day in a specified date range.
Insulin doses are only available if logged in the meter.
4. Basal Titration

The Basal Titration report compares fasting blood glucose levels with long-acting insulin doses over a 12-week period. The report also shows when long-acting insulin doses have been adjusted based on fasting blood glucose trends.
5. Mealtime Patterns

The Mealtime Patterns Report compares the before-meal and after-meal blood glucose levels for breakfast, lunch and dinner, over the specified date range. This report also shows before-and-after meal averages and logged insulin.
6. Dose Plan

The Dose Plan shows you the current meter setup along with the history of settings for spot check therapy and confirmation programming. Insulin settings are displayed visually to reinforce how the patient finds their dose on the meter.
Note: Patients should always consult their healthcare professional if they have any questions or concerns about their diabetes management or results.

Instructions
Simply download the FreeStyle Auto-Assist Neo Software, connect the yellow USB cable to your FreeStyle Optium Neo and your computer. The software will automatically begin the installation process. Compatible operating systems: Windows XP, Windows 7, Windows 8. Read the User’s Guide for further info.

Creating, printing and saving reports


Step 1

Create a report
Once the software has installed, you arrive on the MENU page, connect the meter and click Create Reports.
WARNING: To avoid the possibility of electric shock, never perform a blood glucose test while the meter is connected to the computer.
To create reports for this meter:
- Tick the box next to the reports you want to view or print
- Choose the timeframe you want to show data for, using the dropdown box or by entering a custom range. You can select the last 1 to 4 weeks or you can select a custom start and end date.
- Click Print Reports or View Reports on the screen.
The report will appear on the screen automatically. To select other reports, click Back.
Work with your healthcare professional to understand your report data.
Step 2

Printing a report
To print a report, make sure you've connected your meter and clicked Create Reports.
WARNING: To avoid the possibility of electric shock, never perform a blood glucose test while the meter is connected to the computer.
To print reports for this meter:
- Tick the box next to the reports you want to print
- Click Print Reports
All pages for the selected timeframe will be printed. To limit the number of pages, adjust the timeframe.
You can also print a report directly when it's displayed on the screen by clicking the print button.

Step 3

Saving a report as a PDF
To save a report as a PDF, make sure you've connected your meter and clicked Create Reports.
WARNING: To avoid the possibility of electric shock, never perform a blood glucose test while the meter is connected to the computer.
To save reports for this meter as PDFs:
- Create the report you want to save following Step 1.
- Click Save.
- Choose where you want to save your report, give it a name and click Save.
You can then send the report as an e-mail attachment to your Healthcare Professional.
All pages for the selected timeframe will be saved. To limit the number of pages, adjust the timeframe period.
